By completing the consultation form, you are giving your consent for BODY WISDOM ALCHEMY to hold your personal information. The information which you supply on the form will be held together with follow up notes. I hold your information to give you the best possible service and for insurance purposes.
On the initial consultation form you will be asked to confirm that you have read and agreed to the terms and conditions as well as the privacy policy.
Email Marketing
With your permission, I will send occasional emails about my practice, including special offers, intuitive wellness tips, and seasonal updates. To receive these, simply opt in on your initial consultation form. You can unsubscribe anytime via the ‘Unsubscribe’ link in the emails or by emailing me directly.
Retaining and Storing Your Information
You have the right to ask me to anonymise your records at any time if you wish to terminate our working relationship. We will hold your data for 7 years following our last date of contact and will destroy them with a shredder after that date.
Your information is stored on an encrypted computer, in my password protected online booking system. I do not store personal data on any portable devices such as a memory stick. All documents and apps that contain personal data are password protected.
Protecting your information
I will never share your information without your permission. It will be kept strictly confidential unless there are legal grounds for me to disclose something which you have told me to a third party. I shall always inform you if I feel that disclosure is necessary and seek your permission and co-operation first, including for medical purposes.
